All participants must complete a Waiver & Release Form before taking part in activities, leagues, camps, tournaments, or events at The Soccer Palace.
To save time during check-in, please complete the form below before your scheduled activity. Participants under the age of 18 must have a parent or legal guardian complete and sign the waiver on their behalf.

SOCCER PALACE – RELEASE AND INDEMNITY AGREEMENT
The following waiver applies to adult participants and parents or legal guardians completing the agreement on behalf of a minor.
Important: The complete legal waiver should be displayed in this section, preserving the language of the existing Soccer Palace Waiver Form you provided.
The agreement addresses the following:
Assumption of Risk: Acknowledgment of the risks associated with participation in Soccer Palace activities, including potential injury.
Release of Liability: Release of Soccer Palace and other designated parties from certain claims arising from participation.
Indemnification: Agreement to indemnify the designated released parties as specified in the waiver.
Medical Authorization: Authorization for necessary medical treatment at the participant’s or parent/guardian’s expense.
Photography and Video Consent: Permission for Soccer Palace to photograph, record, and use participant images and likenesses as specified in the agreement.
Governing Law: Agreement governed by the laws of New Jersey.
Jury Trial Waiver: The existing agreement includes an express waiver of trial by jury.
